Premier is the network of dentists for Payors, and we do not have this information. Log in to the provider portal to view the Payor List for phone numbers of where to call for benefits and eligibility.

Log in to the provider portal to view the Payor List of all Payors that use the Premier network.

Log in to the provider portal to view the Payor List for claim payor ID's and/or addresses for mailing claims.

Always submit claims using your office fees. This helps us determine fees for the following year. Submitting claims with Premier fees does not allow us to see discount levels, which can negatively affect your reimbursement levels.

If you feel a claim has not been processed correctly e-mail the EOB to networksupport@sunlife.com. We will help you resolve your claim issues.

Fee schedules are evaluated for an update yearly. We will notify you in November of each year and you can access the latest fee schedules on our provider portal at provider portal

Start by logging in at provider portal

  1. From the homepage, select 'PRACTICE TOOLS' in the top menu.
  2. When this menu opens, you will be able to select 'Fee schedules' under Billing & payments.
  3. On the Fee schedule search page, select Premier Dental Group from the network drop-down and complete the provider and location.

If the dentist practices under the same TIN, they just need to complete a credentialing application. If they practice under a different TIN, we need a contract, application & W-9. Forms can be found online at provider portal.

You will need to complete the Provider Update Form that is available at provider portal and then email the completed form to sunlife.provider.updates@greatdentalplans.com.
